Whenever I try to do a level, solo or with other people, the game starts and then it says "ASSERT FAILED!!" and I disconnect. What's going on?!?!

I was playing on PvP with a couple of people and someone named Darksoulz98 went into trans and had an extended trans, and the "Trans" circle thingy stayed there, not sure if this is a hack or not.
