Just defeat the boss on the halloween map until he drops the box.

It's part of the event if you haven't tried it yet. You collect candy from the event map by killing the bosses. He also has a slight chance of dropping a miasma head.
Receiving candy gives you a random item and for every 5 candy you send you get a reward as well which can be anything ranging from armor, coin items, or a rare limited item.

I ment about the candy things people need to sent you 10candies, and than you get a Miasma Perm ?
naw, the miamsa drops in seperate box, its blue...
