Did you examine Jack, while he was in the room, and then after that Jack left the room? The examiner does not auto update. For example if Ross was to make a room with just him in it, and you were to examine Ross it would say he is in that Room, when Ross leaves that room the room list will update for you showing that there is no longer a room there, but since you still have the examiner page open, it still says Ross is in that room, as it does not update.
