To add an image from a URL, you click the little Earth in the bottom left hand corner, next to the checkerboard stuff.  To add one from a file, you click the floppy disk.

I obviously can't show you anymore because the bots forums are down, but I made tons of guild userbars.  Made one for leader, co-leader, member, ally, etc.  This website made it super easy.

I use UB Designer every time I make a UB and occasionally I'll edit part of it in GIMP for things like recolors.
